24 hour notice is requested if you need to cancel an appointment.
If you miss any appointment without 24 hour notice, an office call fee will be assessed.
This fee cannot be billed to insurance companies and will become the responsibility of the patient.
If an in-office surgery is missed without 24 hour notice, a $50.00 fee will be assessed.
